Declaration Forms

As Singapore is one of the areas in Asia affected by the H1N1 Flu Virus, Character Montessori is taking some pre-cautionary measures at our centres.

As of 22 June 2009, we will be having a few declaration forms for parents/ CM Staff to fill up when they go to affected areas for visitation or holiday. We thank you for your co-operation and for taking time filling up the form.
